Berserker Droid

this was a term developed during the height of the Old Republic to indicate any droid that seemed to ignore its programming and go into a violent fit when it encountered a situation that it deemed threatening. The level of violence differed between types of droids and individual units, but one thing remained constant: something in the hardware or software was not quite right, and the normal threat-avoidance routines did not get activated correctly. Many times, the initial occurance of this aberrent behavior was ignored or dismissed by the droid's owner, who simply had the droid's memory erased in order to eliminate the "glitch." However, the glitches that resulted in berserker behavior were often unaffected by the memory wipe, which added another layer of incoherence to an already-flawed droid. Over time, these droids abandoned their programmed role and took up less lawful endeavors, until their programming simply imploded and they became killing machines.
